  1. How long are the rates guaranteed?
  2. How is the Emergency Room covered?
  3. Is Urgent Care covered?

  4. Are Air and Ground Ambulance covered? How much?
  5. What is the Lifetime Limit of the policy?
  6. Is the Lifetime Limit of the policy per person or per policy?
  7. What type deductible does the policy have? Individual? Annual?       Per incident?

  8. Will the policy cover me anywhere in the United States "In       Network" the same way I am covered in my own state?
  9. What if I move to another state?
10. Does the Network cover Mayo Clinic, MD Anderson, Sloan       Kettering,  and almost every hospital and doctor in the United       States?

11. Do I need a referral to use Mayo Clinic?
12. How is organ transplant covered? Single or multiple organs?
13. Where would I go for a transplant?
14. Is there a companion expense plan available with transplants for       someone to accompany the insured?
15. What about guaranteed conversion for my children?
16. May my children stay on the policy past age 25 if they are not       attending  school but still live at home?

17. How will the insurance co-pay multiple surgeons for single       operation?
18. What are the company's financial ratings? (AM Best and Standard       & Poor)
19. Do I have to ask permission to see a Specialist?
20. What is the maximum limit on the co-insurance? (stop-loss)
21. How much total out-of-pocket per year per person? Per family?
22. What are the limitations in ICU? Is there a maximum amount       covered?
23. Does the contract read "charges will be paid usual and customary       as determined by my doctor, not by the insurance company" and       NOT reasonable and customary?

 

Why is it that many people who are "just visiting" Arizona, decide to move here? There must be a reason that Arizona is one of the fastest growing states in the nation.
